Description Celiac disease represents inflammatory autoimmune disorder of small intestine caused by gliadin with increasing incidence worldwide. Despite vhe existence of modem diagnostic methods celiac disease remains underdiagnosed. In patients without IgA deficiency, the determination of IgA antibodies against tissue transglutaminase represents the main approach in laboratory screening of patients with suspicion of cellar disease. Detection of IgG antibodies against deamidated gliadin peptides is useful for diagnosis of celiac disease in small children. Nevertheless, some patients with celiac disease do not express the typical serological pattern as we show in following three cases of patients with high suspicion of celiac disease proven by biopsy of small intestine. They had negative IgA antibodies against tissue transglutaminase, but positive in IgG class. In addition, the first patient, unlike the other two, didn't have genetic predisposition to celiac disease (HLA-DQ2, HLA-DQ8). Negativity of IgA antibodies against tissue transglutaminase does not necessarily mean either exclusion olCeliac disease or diagnosis of gluten sensitivity.
