Description There were compared anterior and posterior approaches in th treatment of right idiopathic thoracic scoliosis, managed by fusion and segmantal instrumentation. The posterior group compreise 31 girls, with an average age 14,5 years. The curves were improved from 54,3 to 18,7 degrees. Thoracic kyphosis was changed from 30,0 to 20,5 degrees. the average hosptia stay lasted 10,2 days. The anterior approach was used in 25 girls and 6 boys, with an average age of 15,3 years. Thoracic curve was improved, on the average from 53,7 to 23,6 degrees, thoracic kyphosis from 21,5 to 28,5 degrees, the average hospital stay lasted 15,5 days. In anterior group we found lower costs, shorter operative time and lower intraoperative blood loss, but higher postoperative drained blood loss and longer hospital stay - all differences were statistically significant at 0,05. všechny rozdíly byly statisticky významné. The majority of idiopathic scoliosis is indicated for posterior approach, dné zadní výkony, the anterior one is indicated mainly in significant thoracic hypokyphosis or lordosis.
