Description The case report describes a patient who was sent to our department due to anisocoria that was present for one year. The patient has already gone through different examinations, however none of them has led to the cause of anisocoria. Except for anisocoria the patient did not have any other problems. The pupil on the right eye was more than 1 mm larger than the pupil on the left. Direct photoreaction was present on both eyes and there was a dilatation deficit on the left. A slight ptosis of the upper eyelid could be observed on the left. The ocular findings were otherwise normal, both in the anterior and posterior eye segment. The findings corresponded to Horner syndrome left. The case report discusses problems connected with pharmacological tests for Horner syndrome because the standard cocaine test, recommended by most textbooks, is no longer easy to perform as the availability of cocaine is very limited.
