| Description | Monitoring plasma drug concentrations is an essential aspect of contemporary personalized pharmacotherapy in psychiatry. Clozapine (CLO) is an atypical antipsychotic drug often used for treatment of pharmacoresistant schizophrenia. For clozapine, therapeutic drug monitoring (TDM) is highly recommended as optimal therapeutic range is quite narrow, and there is a significant risk of adverse drug reactions. Good compliance is a critical aspect of schizophrenia management because a lack of treatment adherence results in therapeutic failure or inappropriately resolved adverse effects. TDM can help to disclose such situations, properly set dosage and alert to drug interactions. Dried blood spot (DBS) method nowadays is gaining importance in analysis of antipsychotic drugs (LC-MS based). Favorable characteristics of DBS method are low capillary blood volume (10-50 ul) taken from a fingertip or earlobe. The sampling is also less expensive than standard blood sampling with better analyte stability compared to the liquid samples. The aim of this study was to rise to the challenge of characterizing the relationship between clozapine DBS and serum/plasma concentrations. LC-MS method was utilized for CLO concentration monitoring in both matrices (DBS and serum). LC separation was performed on Kinetex C18 column (100x3mm, 2.6 um, Phenomenex) by gradient elution. Optimization part concerned mainly sample preparation procedure – type of sampling, proper device and extraction conditions. Validation of the method comprising alternative blood sampling has met EMA criteria. Finally, strong correlation between serum and DBS capillary blood concentrations was observed. Thus DBS method used for blood sampling could be considered as a feasible approach for CLO monitoring. |
