Description The use of DRG (Diagnosis-Related Groups) classification system for determination of reimbursement for inpatient care is still being expanded; perspectively it is likely to be used for reimbursement of outpatient care too. A thorough knowledge of the DRG as reimbursement mechanism is a prerequisite for the execution of any cost analysis in the Czech Republic. Currently the DRG system may be used in the Czech Republic for inpatient care cost calculations in different types of economic evaluations. Compared to the previous reimbursement mechanism, the DRG system provides a substantial simplification especially for studies conducted purely from the perspective of a healthcare payer and studies examining the direct impact of the observed interventions on costs or savings in the health insurance system. The use of DRG as a single tool for calculating the costs in inpatient care must be chosen very carefully, it is always necessary to consider the adequacy of its use in the planned study design. In this article we also provide a summary of the main principles of the DRG reimbursement system, the current state of implementation of DRG in the Czech Republic, and the options of its use in cost studies are demonstrated on specific examples.
