Popis Osseous involvement represents a critical but often under-recognized component of advanced pressure ulcers. This expert commentary addresses the conceptual and diagnostic challenges related to the classification of bone exposure, osteitis, and osteomyelitis within deep pressure ulcers. Current international staging systems insufficiently distinguish between these entities, which may lead to diagnostic ambiguity, inconsistent documentation, and suboptimal clinical decision-making. Drawing on surgical, radiological, and microbiological perspectives, the authors emphasize the need for a more nuanced approach integrating anatomical depth, tissue viability, and infection status. Clarifying the terminology and classification of osseous involvement is essential for accurate staging, multidisciplinary communication, and optimization of timing and outcomes of reconstructive surgery.
