Exploiting Phage–Antibiotic Synergy to Improve Outcomes in Chronic Staphylococcus aureus Wound Infections

Popis Objective: Phage–antibiotic combinations can enhance antibacterial efficacy via phage–antibiotic synergy (PAS). This study aimed to evaluate PAS across a panel of Staphylococcus aureus clinical isolates and to assess its therapeutic potential in a chronic wound model. Methods: Kayvirus and Rosenblumvirus phages were combined with different antibiotics. Antibacterial activity was evaluated by optical density, viable counts, and microdilution assays. PAS patterns were analysed alongside bacterial and phage genomic data. Results: PAS was observed across multiple phage–antibiotic combinations, with effects strongly dependent on treatment concentration. No consistent association with specific bacterial or phage genomic features was identified. Combined treatment significantly improved antibacterial efficacy in the chronic wound model. Conclusion: Phage–antibiotic synergy is a promising strategy against S. aureus infections, including those in chronic wounds. However, its effects are highly context– and concentration–dependent, and antagonistic interactions may occur. The underlying mechanisms remain largely unresolved. This study identifies effective combinatory treatments and highlights the need for deeper mechanistic understanding to support clinical translation.
