Description Pathophysiological rationale is presented for combining angiotensin converting enzyme (ACE) inhibitors with AT1 receptor blockers for angiotensin II (sartans) in the treatment of hypertension, heart failure, post-myocardial infarction patients, and in the secondary prevention of coronary heart disease (CHD). As yet, there is no clear indication for the use of this combination – no large clinical study has been conducted in hypertension patients, although it/one? is recommended for hypertension associated with significant proteinuria. The combination is not recommended in acute myocardial infarction patients or in secondary prevention of coronary heart disease. In heart failure, this combination is recommended only for cardiologic clinics focusing on heart failure in patients who cannot tolerate aldosterone antagonists.
