Description Colorectal cancer is one of the leading types of cancer in the Czech Republic and worldwide. The palliative treatment of an unresectable disease has developed remarkably over the past decades. New agents in the clinical practice include the angiogenesis inhibitors and EGFR pathway inhibitors. Thanks to new treatment regimens, some patients’ disease becomes resectable from an originally unresectable state. Using these new drugs, the median overall survival has reached 30 months in the metastatic disease. The first and second line of systemic treatment is complex, offers many possibilities. The choice is a matter of the clinical condition of the patient, his or her previous treatment history, comorbidities, the activity of the disease itself, the side effects of chosen drugs, the therapeutic goals and the patient’s wish. Last but not least is the matter of the reimbursement, mainly the public medical insurance conditions. The article offers an overview of the second line systemic treatment, the key clinical trials and some practical aspects that may be relevant in every day clinical practice.
