Description | Occurrence of varicose veins is frequent.25 % of European population. It is linked to the insufficiency of the superficial venous system caused by the incompetence of the great saphenous vein (GSV) and its tributaries, respectively the small saphenous vein. The surgical treatment of varicose veins aims to remove the venous reflux of superficial veins. Contemporary diagnostic methods allow us to reliably diagnose the incompetence of individual valves of the superficial and deep venous systems; surgical techniques have been described in detail and complications at varicose vein surgeries are very rare. Treatments of GSV with minimum invasion have grown in popularity in recent years and this applies globally as well as locally. Endovenous techniques have to a larger or smaller extent - replaced conventional surgical treatments mainly in early stages of CVI. The results are mid-term and experiences with these methods are very good. However, detailed knowledge of the principles of endovenous therapy is essential, including pre-operational sonography. It is always necessary to consider the possible thermal damage to surrounding structures, to apply a sufficient amount of J/cm of the vein length and at the same time to avoid possible burns. Patients report less post-operational pain and smaller limitations to their physical activities.In May 2011 the European Society for Vascular Surgery published its guidelines on the treatment of varicose veins.These set clear procedures for the treatment of varicose veins for the individual sections of the venous system. The guidelines use EBM to classify the safe and effective treatment of GSV with endovenous ablation as 1B, it has been proven that following such treatment morbidity is lower than in the case of a traditional surgery which was classified as 2B. Therapies that involve minimum invasion such as the thermal ablation of the superficial vein system has changed and improved the treatment of varicose veins. |
