Description In the last decades of the last century, the fate of cancer patients has improved significantly, with long-term remissions reaching more than 80% in developed countries. However, the first two decades of the new millennium have witnessed some stagnation in improving overall survival and despite a better percentage of remissions achieved in a number of areas such as metastatic sarcomas, H3K27M mutated midline gliomas or some ependymomas, a significant percentage of patients remain associated with severe acute and long-term toxicity. In adult oncology, the immunotherapy strategy is a clear breakthrough. In children, this development is more difficult to reproduce due to the large spectrum of malignancies and low incidence. In recent years, the knowledge of cancer biology has progressed significantly, which has led to the identification of tumor characteristics that give the tumor cell and tissue a growth advantage. We have been trying to influence these aberrations over the past few years by offering carefully examined patients a treatment based on the presence or absence of certain aberrations or biomarkers.
