Description Aims: The aim of the presented work is to study improper positioning of a patient during taking a cephalogram and its impact on the outcome of cephalometric analysis, as well as to find the parameters influencing biased cephalometric analysis results. Material and method: 10 skulls were scanned at 27 positions. Radiographs were taken in 1 basic position, 8 positions with rotation to sagittal plane (5°, 10°, 15°, 20°), 8 positions with transversal inclination (5°, 10°, 15°, 20°) to sagittal plane, 2 positions with inclination in sagittal plane (20°) to horizontal plane, and 8 positions with combinations of the given rotations and inclinations. All radiographs were taken toward the side of x-ray generator as well as toward the side of x-ray detector (film). 270 radiographs were made and subsequently analyzed. Analyses results were compared and the degree of bias of cephalometric analysis outcomes was evaluated. Results: During skull inclination in sagittal plane a statistically significant different value (p < 0.05) was recorded in the minimum of 0 and the maximum of 2 parameters. During transversal inclination of skull to sagittal plane on the generator side and on the detector side statistically significant different values (p < 0.05) were recorded in the minimum of 1 and the maximum of 10 parameters depending on the degree and direction of the inclination. During rotation toward generator and toward detector statistically significant different value (p < 0.05) was recorded in the minimum of 2 and the maximum of 13 parameters of cephalometric analysis. In the combination of inclination and rotation statistically significant different values (p < 0.05) were found in the minimum of 3 and the maximum of 10 parameters depending on the degree and direction of inclination and rotation. Angular parameters were affected most frequently. Conclusion: In the deviation from the standardized position of patient‘s head in cephalostat the values of cephalometric analysis are influenced by the type of movement, degree and direction of movement, and also by the set of parameters included in a given analysis. Rotation affects outcomes error rate more significantly than transversal inclination; the most critical is the combination of skull inclination and rotation. Angular parameters are affected more than linear ones. However, in evaluating cephalometric analyses results, all types of inaccurracy should be considered. (Ortodoncie 2021, 30, No. 3, p. 155–167)
