| Description | The paper presented here deals with possibilities of a form teachers work evaluation. To do so, it uses examples of questions and answers from research questionnaires that reflect teachers, parents and pupils opinions concerning the educational activity of form teacher. The special attention is given to form teachers activities aimed at strengthening pupils mental, physical and social health. The study summarizes the findings of the research.
