Description Treatment remains one of the current problems when dealing with SCM, particularly of the mild and moderate forms without rapid progression. No good evidence exists that the decompressive surgery with anterior or posterior approach can improve the clinical outcome of the victims of this disease. This uncertainty persists despite the fact that decompression is a logical answer to the stenotic process and that this approach is supported experimentally. Author presents contemporary knowledge and his own results from studies about this challenging task. The article summarises some recommendations how to make decision in individual patients.
