Description A substantial portion of pleural effusions are septated oř loculated. Loculated pleural effusions usually require a partially different diagnostic and therapeutic approach. Loculation most frequently develops in parapneumonic pleural effusions oř empyernas, but it is also frequently observed in malignant effusions, tuberculous pleurisy and hemothorax. The authors present a review of current knowledge on pathophysiology of the development of locu-lation, along wlth current recommendations for the diagnosis and treatment of septated pleural effusions. More-over, they discuss the consequences of treatment failure represented by several types of pleural fibrosis. Finally, they summarize current recommendations for the diagnosis and treatment of the most frequent pathophysiologi-cal types of loculated pleural effusions.
