| Description | Epilepsy is a common chronic neurological disorder treated primarily with antiseizure medications (ASMs). Therapeutic drug monitoring (TDM) is an important tool for optimizing therapy and addressing interindividual pharmacokinetic variability. Conventional TDM requires venous blood sampling, whereas dried blood spots (DBS) offer a minimally invasive alternative created by applying a precisely measured amount of capillary blood onto a collection card, enabling simplified storage, transport, and potential for home-based sampling. The correlation between DBS and plasma concentrations of selected ASMs was evaluated. Lamotrigine, lacosamide, levetiracetam, and valproate were quantified using a UHPLC–MS/MS method. DBS samples were prepared from 45 µL of capillary blood collected on Whatman 903TM cards and extracted using a methanol/acetonitrile/water mixture containing isotopically labelled internal standards. Analysis was performed using a Dionex UltiMate 3000 RS system coupled to an AB Sciex QTRAP 6500+ mass spectrometer. The study demonstrates the potential of DBS as a practical alternative matrix for ASM monitoring. Its implementation could improve patient comfort, facilitate repeated sampling, and increase the accessibility of TDM, particularly for patients requiring long-term follow-up. |
