| Description | Therapeutic drug monitoring (TDM) of antiseizure medications (ASMs) remains essential for optimizing epilepsy treatment and enabling individualized dosing. Conventional TDM is primarily based on plasma or serum analysis; however, alternative sampling strategies offer substantional advantages in terms of patient comfort, accessibility, and suitability for decentralized testing. In this context, dried blood spots (DBS), saliva and dried saliva spots (DSS) represent minimally invasive matrices. Notably, saliva is considered to reflect the unbound (pharmacologically active) fraction of ASMs, offering a non-invasive surrogate for free drug concentrations. In parallel, the determination of the unbound fraction in plasma ultrafiltrate provides an important reference for assessing drug exposure. This study aimed to develop and validate a sensitive LC–MS method for the simultaneous determination of levetiracetam (LEV), lacosamide, lamotrigine (LTG), cenobamate, carbamazepine, eslicarbazepine (active moiety), and valproic acid (VPA) across DBS, saliva, DSS. The plasma ultrafiltrate analysis was additionally evaluated for LTG and VPA. |
